Welcome to the Connexions Health and Social Care Zone!
The aim of this zone is:
- To provide information about the role and responsibilities of Connexions in our work with specific groups of vulnerable young people and in relation to other agencies responsibilities
- To help ensure a clear and effective referral process at the right time for the right young people
- To provide a range of resources and information in one place for Health and Social Care professionals regarding the Connexions Service
- To support joint working
- To improve outcomes for young people
Connexions' primary aim is to provide information, advice, guidance and support to young people to enable them to progress in their lives, and in particular, to increase the number of 16-18 year-olds in education, employment or training (EET) in Nottinghamshire. To achieve this we will work with young people and other agencies to reduce or overcome barriers in their lives and support them to access a range of employment, education and training opportunities appropriate to their needs.
