Connexions role with disabled young people and their parents/carers from 13-19

8-9
(Age 12-14)

The young person is prepared by Connexions PA/school for the process of transition planning through groupwork, individual interviews and use of APIR (Assessment

Planning Implementation and Review) adapted to the needs of each individual learner.

Parent(s)/carer(s) receive information from Connexions about the process of transition planning, clarifying the role of all involved.

Parent(s)/ Carer(s) receive information about future options.

Parent(s)/Carer(s) are invited to a Parents Meeting and interview with the Connexions PA.

9
(Age 13/14)

Headteacher manages Transition Planning process according to SEN Code of Practice and LA Guidelines and ensures that all relevant partner agencies are involved.

The Connexions Personal Adviser attends the Year 9 Transition Planning Meeting.

Parent(s)/Carer(s) invited by the school to attend the Year 9 Transition Planning meeting.

(Age 14/15)

Connexions PA as process manager for the Transition Plan periodically reviews progress with the young person and with partner agencies and initiates remedial action where necessary.

The young person has the opportunity at the annual review (and at other times with PA through APIR review) to review the transition plan and update goals.

Parents have the opportunity to feedback on progress in achieving goals established in transition plan and contribute to ongoing planning.

10 – 11
(Age 14-16)

PA offers advice and information about post-school opportunities, seeks to address barriers to achieving goals, liaising with other agencies as appropriate. The young person explores options post Year 11 and makes applications.

PA ensures applications for options post Year 11 are made in accordance with necessary timescales.

PA ensures young person knows how to access support in post 16 option, introducing to new PA where appropriate.

PA carries out Section 140 assessment with young people likely to leave school at the end of Y11 for FE and training.

PA ensures Section140 assessment is shared with college/training provider and LSC.

PA liaises with Social Worker to ensure APIR informs ongoing Social Care assessments.

Parent(s)/carer(s) visit options under consideration and feedback views.

Parent(s)/Carer(s) ensure they are clear about how options post Year 11 will be put in place and how to access ongoing support.

12 – 14
(Age 16-19)

PA carries out S140 in learner’s final year at school where young person is likely to move onto FE or training and ensures it is shared with future provider and LSC.

PA shares transition plan and other information with future provider, with young person’s permission.

PA introduces leavers to new PA where young person is moving on to college or training or to the PA/other Connexions staff who will support those seeking employment.

Young person continues to explore future options and visit possible future providers. 

Young person makes applications in accordance with necessary timescales.

Parent(s)/Carer(s) continue to support transition planning and explore future options.

Post 16 Learning Option

PA ensures needs as assessed in S140 are being met.
